Please bring a signed copy of the “HIPAA Policies/Agreement”and “Practice Policies/Agreement” to your first appointment. Note: these forms should be signed by each adult attending the session.

Unless I’ve specifically asked you to do so, there is no need to bring either of the “Permission to Release Information” forms.
